Are Ozone and UV CPAP Cleaners Safe? What the FDA Actually Said

Ozone and UV CPAP sanitisers are heavily marketed and not FDA authorised for this use. Here is the regulator's position and what to do instead.

If you have a CPAP machine, you have seen the ads. A sealed bag or a small box that promises hospital-grade sanitising in a few minutes, no water, no drying, no scrubbing. It is a genuinely appealing pitch, because washing a hose is tedious and nobody enjoys it.

The regulator’s view is less enthusiastic.

What the FDA concluded

The FDA determined that ozone and UV-based CPAP cleaning devices are “not legally marketed for this use” and that “it is unknown if these CPAP cleaners are safe and effective”1 .

That phrasing is worth reading carefully. It is not a finding that these devices are dangerous. It is a finding that they have not been through the process that would establish whether they work or whether they are safe — and that they are being sold for a use they were never authorised for.

The specific risks named

Ozone. The FDA reported that “exposure to high levels of ozone gas may worsen a patients’ existing chronic respiratory diseases or increase the chance of a respiratory infection”1 .

This is the part that should give CPAP users pause. The population buying these devices is, by definition, a population with a breathing-related condition — and frequently with coexisting asthma or COPD. It is a poor match between product and customer.

The FDA documented 11 adverse reports between 2017 and 2019 from patients who experienced cough, difficulty breathing, nasal irritation, headaches, and asthma attacks after ozone-based products were used1 .

UV light. The agency noted that “UV light-based products could cause burns, eye damage or increase the risk of skin cancer due to over exposure”1 .

There is a second, less discussed issue with UV: it only disinfects what the light physically reaches. The inside of a six-foot corrugated hose is exactly the geometry UV handles worst, and it’s the part most likely to be damp.

What to do instead

The FDA’s own recommendation is that consumers and healthcare providers “follow the cleaning instructions provided by the CPAP manufacturer,” and that “these directions normally include regular cleaning with soap and water”1 .

The counter-argument, fairly stated

The honest case for sanitisers is compliance: a person who runs a three-minute cycle every day may keep their equipment in better shape than a person who intends to wash it weekly and does so twice a year. That is a real argument and it applies to a lot of health behaviours.

It doesn’t survive the specifics here, for two reasons. First, the daily task that matters most — wiping the mask cushion so skin oil doesn’t break your seal — takes thirty seconds and a sanitiser doesn’t replace it. Second, the risk profile is aimed squarely at people with compromised airways. A convenience benefit has to be weighed against the FDA’s specific warning that ozone exposure may worsen existing respiratory disease1 .

If washing weekly genuinely isn’t happening in your life, the better answer is to shorten the routine — wipe daily, wash the hose fortnightly, replace consumables on schedule — rather than to buy an unauthorised device.

What about products that claim FDA clearance?

Some manufacturers point to registrations or clearances that don’t mean what the marketing implies. FDA registration of an establishment is not the same as clearance of a device for a specific use, and a clearance for one purpose is not a clearance for CPAP sanitising.

If a product claims authorisation, the specific thing to ask is: cleared for what indication, and under what number? Vague claims of “FDA registered” should be treated as marketing rather than as regulatory approval.
